Vitamin K is a dynamic nutrient that comes in two forms that display unique properties and play important roles in bone health: vitamin K1 (phylloquinone) and vitamin K2 (menaquinone). What many people don’t realize, however, is that vitamin K2, the menaquinones, appears in at least 15 different forms! In relation to bone health, the two most well-studied menaquinones are MK-4 and MK-7[1].

Each of these forms of vitamin K has a unique chemical structure and, therefore, its own activities, unique benefits and uses.

In this article, we will focus on these two different forms of vitamin K2 and discuss how each form is metabolized within the body and the proper dose of each likely to provide benefit for our bones.

Vitamin K2 and Bone Health

Vitamin K1 is well-known for its role in activating the clotting factors that prevent us from bleeding out from even the tiniest paper cut. Vitamin K1 also helps lower inflammation; that’s the role in which this form of vitamin K supports bone health. Vitamin K2, however, is the primary form in which vitamin K is a crucial nutrient for our bones because K2 regulates the healthful use of calcium within the human body[2].

Specifically, vitamin K2 helps with bone mineralization by activating a protein called osteocalcin, which binds calcium and ferries it into our bones. Osteocalcin is secreted by osteoblasts, our bone-building cells. Once activated, osteocalcin helps bring calcium into our bones, enhancing bone density and strength.

Vitamin K2 also prevents calcium from depositing where we don’t want it. Vitamin K2 does this by activating another protein called Matrix-Gla protein (MGP), which prevents calcium from depositing in soft tissues, like the blood vessels in our cardiovascular system and kidneys, and protects them from calcification and stiffening. As such, this protein, once activated by vitamin K2, provides protection against cardiovascular and kidney disease[3][4].

Working in tandem, osteocalcin and MGP ensure calcium is delivered into your bone matrix and kept out of your heart, blood vessels, and kidneys. None of these activities can be accomplished without the help of vitamin K2[5].

The Two Forms of Vitamin K2: MK-4 vs MK-7

As mentioned previously, the two best-studied forms of K2 are MK-4 and MK-7. While these two nutrients are both menaquinones, there are some significant differences in their structures that impact how long they remain available in your body. Let’s dive in.

Role of Vitamin K2 MK-4

MK-4 is smaller in structure than MK-7 and is actually almost identical in structure to vitamin K1. Both MK-4 and KK-7 are used for activating blood clotting proteins before they are used for bone health.

Once all your blood clotting needs are taken care of, any leftover MK-4 can be packaged up into triglycerides and used for bone health (activating osteocalcin and MGP) – but there’s a catch.

Triglycerides don’t stick around in your blood for very long; they are either rapidly used up for fuel or stored in your fat cells. As such, MK-4 gets cleared from your blood within six to eight hours maximum.

Role of Vitamin K2 MK-7

Like MK-4, MK-7 will be used first to activate blood clotting factors if that need exists. Leftover MK-7 will then move on to support bone health.

Although both MK-4 and MK-7 can activate osteocalcin and MGP, MK-7 remains available to support the healthful use of calcium in your body far longer than MK-4. While MK-4 is packaged into quickly used or stored triglycerides, due to its larger structure MK-7 is packaged into cholesterol, in which it travels around your body for several days. This not only gives your bones more time to utilize the MK-7 you get from food or supplements, but it also enables your body to accumulate a “savings account” of MK-7 that you can always draw from to keep your osteocalcin and GLA matrix proteins firing at all times.

Which Is Better for Osteoporosis, MK-4 or MK-7?

Because MK-4 is cleared from the body in 6-8 hours, it must be taken several times each day and in far larger doses than MK-7. Because of MK-7’s far longer availability, much smaller doses provide constant support for osteocalcin and MGP activation. For this reason, MK-7 is the superior form of vitamin K2 to support bone health, especially through the lens of osteoporosis.

Osteoporosis occurs when your bones lose their mineral density and become weak and brittle. As you just learned, vitamin K2, particularly MK-7, plays a crucial role in enhancing bone mineral density by increasing calcium uptake[6][7].

Bone mineralization must be in healthful balance with bone breakdown for someone looking to overcome osteoporosis or even the beginning stages of osteopenia. This means that providing your body with a steady stream of nutrients to enhance bone health is essential.

With MK-7, your body will receive a lasting supply of vitamin K2, as opposed to MK-4, which you’ll need to replenish several times each day.

Natural Sources of Vitamin K2

Vitamin K2 can be found in a variety of animal food sources, but very few contain enough of this nutrient to support bone health on their own. While K1 can be converted to MK-4 and be used for bone health after your blood clotting needs are taken care of – if your digestive tract is well populated with the health-promoting bacteria that do this job – and certain animal-derived foods, such as pastured eggs, grass-fed butter, meat, and certain cheeses contain small amounts of MK-4, you would have to consume very large amounts of these foods to provide enough MK-4 to reliably support your bones’ health.

The one known food source that contains enough MK-7 to take care of your bone health needs is a Japanese fermented soybean product called natto. Natto contains 380 micrograms of MK-7 in just 1.4 ounces.

Foods that contain vitamin K2 in the MK-4 form include:

  • Egg yolks (1 ounce) 4.4 to 9 micrograms
  • Chicken liver (4 ounces) 14.4 micrograms
  • Chicken breast (4 ounces) 10 micrograms
  • Ground beef (4 ounces) 7 to 9.25 micrograms
  • Meat franks (4 ounces) 11.2 micrograms
  • Sauerkraut (3.5 ounce) 4.8 micrograms

As you’ll soon see, none of these sources would have enough MK-4 to meet your nutrient needs for bone health.

MK-7 and MK-4 Dosage and Supplementation

When assessing the appropriate dose of MK-7 and MK-4 for bone health, we have to keep in mind two things:

#1: What each form is preferentially used for

#2: How long each form stays in the blood and is available to activate bone-building proteins

In the case of MK-4, studies show that you would need 45,000 micrograms a day (or 45 milligrams) divided into 3 doses, one taken every 6-8 hours, to provide benefits for osteopenia or osteoporosis.

In addition to the very large overall daily dose, an amount that would be impossible to consume from food, you would also have to take an MK-4 supplement every six to eight hours to ensure your levels stay high, which would mean three doses of 15,000 micrograms each day.

On the other hand, since MK-7 can accumulate in your body, you need a much lower dose, and it can be taken only once a day. In fact, a dose as low as 100 micrograms per day is typically enough to keep osteocalcin and matrix-GLA protein active.

This is an amount that can easily be supplied by your diet if you eat natto regularly.

However, as natto is an acquired taste that most people living in the West (the U.S., Canada, or Europe) aren’t accustomed to, a more practical option is to take a supplement that contains MK-7.

Takeaway

Both forms of vitamin K2 display a range of beneficial activities in your body, but MK-7 is clearly the nutrient to focus on when it comes to bone health.

This form of K2  remains available in your bloodstream for far longer and is able to accumulate, building up a buffer that ensures a steady supply. This means you can take a much lower dose and still meet your bone-building needs, getting calcium into your bones and preventing calcium from depositing in unwanted places.

Thank you for this. I have tried many brands of MK-7 and cannot tolerate any of them, I get nauseous with diarrhea. So, I am taking MK-4 instead. I also eat a lot of fresh veggies, magnesium, strontium and alaecal. I can’t eat Natto or even soy and am hypothyroid. I am taking 10mg testosterone 3x week. I am eating a slice of Gouda every day and trying to eat more eggs every week.Is this enough or am I missing out in something?

Since MK7 attaches to LDL to move about, would a low LDL number cause less MK7 to circulate in the body? With that low “good cholesterol” number would it be a good idea to take a good K supplement like Thorne’s 3k complete?

  16. Kirby Johnson

    March 14, 2022 , 11:15 am

    Steve,

Thanks so much for your patience while Lara answered your question. Lara is our resident Bone Health Expert and you can learn more about her HERE. You’ll find her response below:

“My hunch is that LDL would need to be VERY low to result in MK-7 inadequacy for two reasons: (1) so little is required — 100 mcg of MK-7 is sufficient for most people, not only because MK-7 remains in the circulation in LDL for several days, but (2) also because the body really values vitamin K and is very good at recycling it. We have a special vitamin K recycling maneuver called the vitamin K-epoxide cycle that restores used up (oxidized) vitamin K to its ready to use again (reduced) form. The recycling of vitamin K epoxide (oxidized/used form) to hydroquinone (reduced/ready to use again form) is carried out by two reactions that reduce vitamin K epoxide (KO) to vitamin K quinone and then to vitamin K hydroquinone.

“The Thorne product is OK; it contains 1,000 mcg of K1 (which has important anti-inflammatory effects that protect bone), and 5,000 mcg of MK-4 (which needs to be taken in a dose of 15,000 mcg every 6 – 8 hours to benefit bone, so this will have little effect). Finally, it provides 90 mcg of the form that works, MK-7, for which at least 100 mcg is needed. Note: In humans, the trans isomer of MK7 is the form responsible for K2’s biological activity and is the form that plays such an important role in bone, cardiovascular and kidney health. Cis-analogs of vitamin K2 are biologically inactive. I’d expect that Thorne product contains trans MK-7, but they do not specify.”

Lara’s comments above should not be treated as medical advice – we always recommend discussing this information further with your doctor. Instead, these comments are offered as a matter of opinion and should be taken as such. Hope this helps answer your question!

Amazing article! Thank you for sharing the information Lara! Should one be taking Vit K2 everyday if they have osteoporosis? Which brand would you recommend?

  44. Manja

    May 3, 2023 , 9:56 am

    Great question Saima!

Vitamin K is fat-soluble, so we don’t have to take it every day because we will store some. We can either take a smaller dose every day or a larger dose once or several times a week for example! The dose would depend on your blood levels, but the current research indicates a daily dose of 100 – 360 micrograms is safe and the Institute of Medicine has set no Upper Tolerable Limit for any form of vitamin K. We don’t have specific brand recommendations, but most people would best benefit from vitamin K2 in the MK7 form.

Is there research for other forms of K7? Or a problem with taking an additional supplement for K2 with the 7?

  89. Manja

    May 3, 2023 , 9:30 am

    Hi Karyn,

There are only two forms of vitamin K2 that are typically found in supplements: MK-4 (synthetic) and MK-7 (natural). For more information, please visit our Vitamin K2 overview article, which includes The Complete Vitamin K Bone Health Series List HERE. Let us know if you have any more questions!

My diet: I eat mainly fruits and vegetables and nuts. Also everyday I eat sauerkraut. Based on an article I read on AlgaeCal I also eat a few prunes each day now that I know how good they are for us. I never drink soda or eat fried food.

My exercise: I belong to OsteoStrong and on my own do 10 minutes of planks a day, over 100 squats a day, 100 push ups (from my toes on the floor), walk an hour everyday and a few other exercises.

Result: Over 28% improvement in my bones.

Thank you for this detailed explanation! I am wondering: If these forms of vitamin K are so difficult to get through diet alone, in quantities adequate to building strong bone, why don’t all humans develop osteoporosis? Also: What is the importance of the role of Vitamin MK4 and MK7 relative to other nutrients for building stronger bones? There are so many suggestions for improving our health by adding foods to our diet and yet it is so true that we can only eat so much in a day!

  211. Manja

    May 3, 2023 , 12:11 pm

    Hi Kay,

Great points! Lack of optimal bone-building nutrients is only one of many contributing factors to bone loss, you can learn more about the most common causes HERE. Not everybody is affected by them in the same way or to the same degree.

In relation to your bones, vitamin K2 directs calcium to your bones and prevents it from building up in soft tissue.

Kay, it’s certainly important to get most of your nutrients from food sources! However, many people aren’t meeting the dietary requirements. In particular, older adults may find it challenging to reach the recommended amounts, which is why it becomes necessary to take supplements.

Is the MK7 sources from soy, and if so, is it non-GMO?

  235. Manja

    May 3, 2023 , 12:29 pm

    Hi Stephanie,

There are a number of sources of vitamin K2 including eggs, meat, and various cheeses. By far, the richest dietary source of K2 is Japanese natto, which is made out of fermented soybeans. It is important to note, however, that not every soy is genetically modified. Japanese soybean farmers do not usually use any genetically modified seeds.
As for the source of K2 in AlgaeCal, our vitamin K2 is derived from plant-based essential oils (rose oil & citronella oil).

I did a webinar with Dr. Greger, MD on K2 last year. He said the research on K2 was flawed and that you can convert K1 into K2 if you eat enough green leafy vegetables. Would you agree that that is accurate information? He said marketing that you must get K2 with D is simply not accurate information. Please let me know your thoughts on this. Thank you, Robin Vinge, ND

  270. Manja

    May 3, 2023 , 12:47 pm

    Hi Robin,

It’s true that your body can convert vitamin K1 into vitamin K2. But there’s a catch; the conversion will only happen if there’s enough vitamin K1 left after it’s been used for its blood clotting and other jobs. If you eat a lot of leafy greens like kale and collard greens, you may consume enough vitamin K1 to have some leftover to convert into vitamin K2. But it would have to be a lot of leafy greens. And for most people, it’s an unrealistic amount. You can read more about this HERE.

I found this very informative, but as I take a blood thinner, I wonder if the K2 in algecal plus is safe to take?

  337. Samantha AlgaeCal

    May 5, 2023 , 7:45 am

We’re so happy you found this information informative, Joanna! AlgaeCal Plus should not be taken with the class of blood thinning medications that are vitamin K antagonists due to the added vitamin K2. There are also other types of blood thinning medications that do not interact with vitamin K, so you may find it helpful to discuss those options with your doctor. Please don’t hesitate to reach out if you have further questions or concerns!

  360. Samantha AlgaeCal

    May 5, 2023 , 10:44 am

We certainly understand that a volume of information that be harder to grasp, Carol! The key takeaway when it comes to vitamin K supplementation is that you want to aim for Vitamin K2 MK-7 form. MK-7 stays in your body for up to 4 days! MK4, on the other hand, only sticks around for 6-8 hours. The fact that MK-7 has a longer half-life means you can build up a steady level when you consume it regularly. You can withdraw from it each day to continuously activate the osteocalcin and matrix Gla proteins to steer the calcium you consume in the right direction… your bones!

I hope this clarifies this information further for you! Feel free to call our Bone Health Consultants at 1-800-820-0184 if you have any questions – they’re super knowledgeable on this topic!

  389. Elizabeth

    May 3, 2023 , 4:31 pm

Can you give an approximate optimal amount of MK7 we should shoot for? Thanks

  390. Samantha AlgaeCal

    May 5, 2023 , 12:14 pm

Great question, Elizabeth! Ideally, men should aim for 120mcg daily and women should aim for 90mcg daily. If you are taking additional D3, then your requirement for Vitamin K2 also increases. These two vitamins work hand in hand to deliver calcium directly to your bone. Vitamin D3 helps absorb calcium into your bloodstream, then vitamin K2 directs the calcium from your blood into your bones. Our suggestion for someone who is taking more than 5000 IU of D3 daily is to ensure that you are getting a total of 200mcg daily of Vitamin K2. Since the daily dosage of AlgaeCal Plus will provide you with 100 mcg of K2 (Mk-7) we recommend that you take an additional 100mcg of Vitamin K2. I hope this helps! Please don’t hesitate to reach out if you have any further questions. 🙂

  439. Nan

    May 3, 2023 , 6:44 pm

I’ve been careful to take vitamins K2 and D3 together, but didn’t know there were 2 forms and that MK-7 is what I should look for. My combination supplement just lists the K2 as menaquinone.
It disturbs me to think that even if one ate a very healthy diet they would not be getting adequate amounts to keep the K2 levels up in their body; almost as if it is an unnatural system.
Where do the other long chain menaquinones stored in the liver come from? Can any of the excess K1 or MK-4 that makes it back to the liver be recycled into these long chain menaquinones?

  440. Chelsea Dugas

    May 9, 2023 , 1:47 pm

Thanks for sharing your questions, Nan! We’ve reached out to our Bone Health expert, Lara Pizzorno, and this is what she shared:

    “Hello Nan,
It’s not an unnatural system. What is “unnatural” is our modern diet and conventional agricultural practices.
Menaquinones are not stored in the liver. In the liver, the long chain menaquinones are packaged into cholesterol and sent inside it out into the general circulation. MK-4, the shortest menaquinone, is, like vitamin K1, put into triglycerides before being sent out into the circulation. The reason MK-7 is more effective than MK-4 is that because it circulates in cholesterol, it is not cleared from the bloodstream for several days, while MK-4, which circulates in triglycerides, is cleared within hours. Because MK-7 remains in the circulation for several days, a far smaller amount of MK-7 is needed (about 100 mcg per day for MK-7 versus 15,000 mcg every 6-8 hours for MK-4), plus MK-7 taken daily builds up a reserve so remains constantly available to activate the vitamin K dependent proteins that regulate our healthful use of calcium (e.g., osteocalcin and matrix Gla protein). For a full discussion of all this, please read the chapter on Vitamin K (K1 and K2) Healthy Bones, Healthy You!
Apart from dietary intake sources, which in westernized countries, the U.S., Europe, etc., are primarily certain cheeses and free-range eggs, the long chain menaquinones (largely MK-9, MK-10) are synthesized by gut microbiota, predominantly in the ileum, the last part of the small intestine. They are not produced from either K1 or MK-4. Which – if any — long chain menaquinones are produced in an individual’s intestine varies greatly due to differences in the composition of different individuals’ intestinal microbiome. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8769504/
    https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9058076/ https://www.frontiersin.org/articles/10.3389/fmed.2022.829304/ https://pubmed.ncbi.nlm.nih.gov/19949269/
While it’s recently been discovered that humans are capable of producing MK-4 in quite a few tissues, providing optimal amounts of this critical nutrient is, unfortunately, not happily straightforward for several reasons:
Producing sufficient MK-4 requires a constant supply of excess K1 since (per Ames’ groundbreaking paper on triage theory, which was published in 2009, https://pubmed.ncbi.nlm.nih.gov/19692494/ ), K1 will first be used to activate clotting factors, and only excess K1 will be available for MK-4 synthesis. As I’ve mentioned, the MK-4 form of K2 clears typically in 6 hours, possibly in 8-10 if your genetic inheritance includes slower versions of the enzymes that clear vitamin K, but again, far from rarely seen SNPs result in an increased rate of vitamin K2 clearance.
Very few people now consume large amounts of leafy greens or unrefined vegetable oils containing phylloquinone, and certainly do not do so every 6-8 hours. Not only do we not ingest the necessary amounts of leafy greens, but the primary source of K1 in the modern diet is processed vegetable oils. Oil processing/refining changes the form of K1 from phylloquinone to a form called dihydrophylloquinone. In humans, dihydro-K1 is poorly absorbed and produces low to no biological activity. https://pubmed.ncbi.nlm.nih.gov/11722960/ Dihydro-K1 is not converted to MK-4, which is why its intake is associated with low BMD and CVD. https://pubmed.ncbi.nlm.nih.gov/10491675/ https://pubmed.ncbi.nlm.nih.gov/17684225/
Next, the enzyme that converts K1 to MK-4 is called UBIAD1, and single nucleotide polymorphisms (SNPs) that adversely affect the function of this enzyme are not uncommon. The most researched such SNP is the G186R mutation. https://pubmed.ncbi.nlm.nih.gov/34813684/ If your genetic inheritance includes a version of UBIAD1 that doesn’t work very well, then you will not be producing much MK-4 even if you have any K1 (phylloquinone) available.
Another problem is the all-too-common use of the bisphosphonates and statin drugs, both of which interfere with the mevalonate pathway and thus the production of a compound called Geranylgeraniol (GG) that UBIAD1 has to use to produce MK-4 from K1. https://pubmed.ncbi.nlm.nih.gov/36557825/ If you are taking one of these medications, your ability to produce MK-4 is severely inhibited.
And one more potential issue is a lack of the specific friendly bacteria in an individual’s microbiome that are needed to produce the long chain menaquinones. Dysbiosis (imbalances among the organisms present in an individual’s microbiome) are also very common, affecting millions of people in the U.S. https://www.webmd.com/digestive-disorders/what-is-dysbiosis#
For all these reasons, despite the fact that I would never take a bisphosphonate, do not take a statin, and do consume unrefined organic vegetable oils and eat at least 5 cups of organically grown leafy greens daily – picked fresh from our own garden — I continue to recommend supplemental MK-7 and personally rely on its use to safeguard the health of my bones and cardiovascular system.
Hope this information helps – and if you are taking AlgaeCal Plus, you can relax. You’re getting 100 mcg of MK-7 daily.”

We are truly sorry to hear that you haven’t yet found a vitamin K supplement that fits best for your body, Alice! While we don’t have a recommendation specifically for a vitamin K supplement, our AlgaeCal Plus HERE contains 100 mcg of vitamin K that may be a good for you :). It not only contains vitamin K, but includes 13 bone essential minerals along with vitamins D, C, and K to help increase your bone density safely and naturally!

Of course, if you’re looking for a stand-alone vitamin K supplement, we recommend asking for a pharmacist or your doctor for their best recommendations. I hope this helps, Alice!

We are sorry for any confusion caused, Rebecca! While we are not medical professionals, please feel free to give us a call at 1-800-820-0184 (US & Canada Toll-free) and we’d be happy to address your questions and concerns to the best of our abilities!

Rebecca, you are correct that AlgaeCal Plus already contains 100 mcg of vitamin K2 per 4 capsules. This is usually sufficient for most people, but some may need to take an extra vitamin K supplement depending on how much vitamin D they are consuming. If you are taking additional vitamin D on top of AlgaeCal Plus, then your requirement for Vitamin K also increases. These two vitamins work hand in hand to deliver calcium directly to your bones. Vitamin D3 helps absorb calcium into your bloodstream, then vitamin K2 directs the calcium from your blood into your bones. If someone takes 5000 IU or more of D3 (AlgaeCal Plus provides 1600 IU) then 200 mcg of K2/MK-7 is suitable. AlgaeCal Plus is providing 100 mcg already, so an additional 100 mcg would be required. However, we would recommend discussing this with your primary care physician or pharmacist before introducing into your routine! 🙂 Do let us know if you have any questions!

If I understand correctly, you need the soy in natto to reap the benefits from taking MK7. But, what if your allergic go soy? Currently, I eat raw veggies at each meal including, at times, breakfast. And, exercising 3-4 days a week is an added incentive.

  526. Yoori AlgaeCal

    May 7, 2023 , 3:51 pm

Great question, Rashidah! If you’re allergic to soy in natto, we recommend getting the amount of vitamin K2 that you need for your bones by taking a supplement :). The amount of vitamin K recommend would depend on the amount of vitamin D someone is taking. If someone takes 5000 IU or more of vitamin D3, then 200 mcg of K2/MK-7 is suitable. Please do discuss this with your primary care physician or pharmacist before introducing into your routine!

It will take some time to grasps all the information. Thank you Lara for helping us being aware and guiding us in the right direction.

Your comment that are soil is becoming depleted of certain vitamins, but it makes sense. I think I may try to grow some of my own leafy vegetables on my patio. One question I do have it how much of the recommended amounts of vitamin K are we getting in our bone builder pack?

  557. Yoori AlgaeCal

    May 7, 2023 , 4:08 pm

Growing your own leafy vegetables sounds great, Caryn! AlgaeCal Plus that’s in the Bone Builder Pack contains 100 mcg of vitamin K2 per 4 capsules. If someone takes 5000 IU or more of D3 (AlgaeCal Plus provides 1600 IU), then 200 mcg of K2/MK-7 is suitable. AlgaeCal Plus is providing 100 mcg already, so an additional 100 mcg would be required. However, we would recommend discussing this with your primary care physician or pharmacist before introducing into your routine! 🙂 Do let us know if you have any questions!

I make my own Natto using an Instant Pot. I find the recipe on You Tube. When it is ready , keep it in the fridge for 24 hours. It help with the taste. Then I freeze in small containers or Ziplock. The best way to eat it is to add 1/2 teaspoon of prepared Dijon mustard with seeds to 2 tablespoons of Natto .
You get accustomed to the taste.
I think it is better to get vitamin directly from the food.
Also I read that it is better to take vitamin K and vitamin D with an interval of 3 hours for optimal absorption . Otherwise as both vitamins are fat soluble they will compete with each other.
It seems that vitamin K can help also with Alzheimer disease.

I’m wondering if this vitamin from fermented soybeans relates to Miso and would be a good source?

  586. Yoori AlgaeCal

    May 7, 2023 , 9:37 pm

Great question, Barb! My understanding is that one tablespoon of miso paste contains around 5 mcg of vitamin K2 only, whereas one tablespoon of natto contains around 150 mcg of vitamin K2. Natto seems to be the only food source that contains vitamin K2 in amounts great enough to make a real difference to your bones! We recommend taking a vitamin K2 supplement if you’re not a big fan of natto :).
